As a courtesy to our patients, we will directly bill your insurance company for medically indicated services.  Once payment is received you will be billed for any remaining co-insurance or deductible.  We prefer co-payments for Office Visits, if required by your insurance plan, to be paid at the time of service.  If you have a supplemental insurance plan we will bill them directly for any co-insurance or deductible amounts.

If your insurance plan requires an authorized referral for services provided by Dr. Urban it is your responsibility to obtain the referral.  We will be happy to assist you if we can.  Please provide us with this information when scheduling your appointment.

For self-paying patients treated emergently, we will forward a statement and expect payment within 45 days.  Arrangements may be made for participation in a payment plan by contacting our Office Manager.

For all elective Cosmetic Procedures, payment is due 21 days in advance.  Cashier checks are required for payments made less than 14 days in advance.

We accept VISA, MasterCard, Discover, American Express and Care Credit for payment of any service.
